This romantic heart and swan wedding theme is what my husband and I based our own wedding celebration on. Symbolically, the hearts represent two hearts joining together, while swans mate for life.
Most of the information given here in regards to colors, attire, decorations and so forth, are what we actually used.
The colors we chose were ivory and navy blue. You could pick your own color combination, such as red (hearts) and white (swans) for example.
For our own wedding, we chose invitations with swans embossed on ivory card stock. You may wish to use heart wedding invitations instead. The choice is up to you. Here are some ideas on how you could make your own invitations using DIY wedding invitation kits:
I purchased small wooden hearts at a local craft store and painted half of them ivory, and the other half navy. These were then decorated with dried flowers and ribbons. A ribbon hanger was attached to the back so you could secure them to the pews and/or end aisle chairs. Additional ideas:
Our heart and swan wedding theme ceremony and reception were held in the same place, so we only needed to decorate once.
We used helium filled ivory and navy blue balloons tied with curly ribbon, which were gathered into large bouquets and placed around the room.
The head table, food serving tables and gift table were decorated with tulle swags and tied up to the table with large blue and ivory ribbon bows that were enhanced with silk flowers, streaming narrow ribbons and string craft beads.
My in-laws created a wedding ceremony arch from thin plywood. They cut out the shape of hearts from the plywood, which looked very pretty. We then wound fake ivy along the outside edge and strung some small white lights through the ivy.
For the centerpieces on our main tables, I purchased large, white plastic garden swan planters. I outlined the "feathers" by gluing strings of ivory craft beads along the ridges on the form. In the center of the swan, I created a bouquet of ivory, navy and dusty rose flowers, and inserted baby's breath and small ribbon bows (attached to wire picks) among the flowers. I then glued navy blue ribbon around the neck of the swan and created a bow in the front, to which I added small flowers, ribbon streamers and beads.
For the guest tables, I painted some wooden hearts in navy and some in ivory. Along the edges, I painted simple flowers of the opposite color and made small curved lines between the flowers in dusty rose. In the center, I glued a painted wooden candle holder and tied a ribbon and beaded bow around the neck edge. Each candle holder held a taper candle either in navy or ivory.
In keeping with the heart and swan wedding theme idea, we used pieces of white and blue chenille pipe cleaners to create swan shapes. The "body" section was pulled slightly apart and we placed mints tied up in tulle in the center of each.

A co-worker was into cake decorating and made a lovely two-tiered round wedding cake with beautiful scalloped edges and wedding bell adornments. Although not really a "themed" cake, we appreciated the gesture.
For serving our guests, we purchased a large slab cake which the catering staff sliced up in the back kitchen. Doing it this way, saved us a fair amount of money.
You could also have a heart-shaped cake with a blue-icing "lake" in the center with two swans "swimming" in the lake. Here are some photos of swan and heart wedding cakes that may work for you:

I created our swan wedding cake topper using an plastic oval base that I bought at a cake decorating shop.
I then glued some ruffled lace around the outer edge and added a wired spray of small white silk flowers to the back side.
We happened to find some beautiful crystal swans at a jewelry store that I purchased. They were set on a mirrored base and trimmed with gold along the edges. I fell in love with them! Each was about 3" in diameter and about 3" high.
These swans fit perfectly in the center of the cake topper I had made and fit our theme perfectly.
